Liability Waiver, Parent Consent & Photo/Video Release

1. Parent / Guardian Consent

I, the undersigned parent or legal guardian, give permission for my child (or children) to participate in the Future Founders Academy program operated by Tori Prince ("the Program"), including all in-person sessions in Hammond, IN, virtual Zoom sessions, guest speaker events, field trips, and Pitch Day activities scheduled from May 12, 2026 through the school year.

I confirm that my child is in good general health and able to participate in the program activities. I will inform the Program in writing of any allergies, medical conditions, dietary restrictions, or accommodations needed.

2. Assumption of Risk & Liability Waiver

I understand that participation in the Program involves activities including, but not limited to, classroom instruction, group exercises, public speaking, optional transportation, and meals. I voluntarily assume all risks associated with my child's participation.

To the fullest extent permitted by law, I, on behalf of myself, my child, and our heirs, hereby release, waive, and discharge Tori Prince, Future Founders Academy, its instructors, volunteers, host venues (including pizza/restaurant location and church location in Hammond, IN), transportation provider, and affiliated parties from any and all claims, demands, damages, or causes of action arising out of or related to my child's participation in the Program, except in cases of gross negligence or willful misconduct.

I agree to indemnify and hold harmless the Program and its representatives from any losses, liabilities, damages, or costs they may incur due to my child's participation.

3. Emergency Medical Authorization

In the event of a medical emergency and if I cannot be reached, I authorize Program staff to seek emergency medical care for my child. I agree to be financially responsible for any resulting medical expenses. I acknowledge that the Program does not provide medical insurance for participants.

4. Code of Conduct

I agree that my child will follow Program rules, treat instructors and peers with respect, and refrain from disruptive, unsafe, or harmful behavior. The Program reserves the right to dismiss any participant for repeated violations, with no refund beyond the standard refund policy.

5. Transportation Release (if elected)

If I have elected door-to-door transportation ($30/week per household), I authorize the Program's licensed transportation provider to transport my child to and from designated pickup/drop-off addresses. I confirm the address provided is accurate and I will provide written notice of any changes.

6. Photo & Video Release

I grant Future Founders Academy and Tori Prince permission to photograph, film, and audio-record my child during Program activities and to use those images, videos, voice recordings, and the child's first name (no last name without separate written consent) in promotional, educational, and marketing materials including but not limited to the Program website, social media, flyers, and presentations, without compensation.

If I do NOT wish my child's image or voice to be used, I will notify the Program in writing at join@futurefoundersacademy.com prior to the first session.

7. Background Check Disclosure

The lead instructor, Tori Prince, has completed a recent background check. Documentation is available upon written request to verified parents/guardians of enrolled students.

8. Refund Policy Acknowledgement

Early-bird pricing of $175 (one student) and $275 (two students) is available through May 10, 2026. The refund cutoff is May 31, 2026: after that date, no refunds will be issued. If cancellation is requested on or before May 31, 2026, $65 is retained as an administrative fee and the remainder is refunded within 14 business days. Weekly food cost ($15/student/week) is not refundable for weeks already attended.

9. Signature

By submitting the enrollment form on the Future Founders Academy website and checking the "I agree to the waiver, consent, and photo/video release" box, I acknowledge that I have read, understood, and agreed to all terms above. An electronic signature has the same legal effect as a handwritten signature.
